How ACOs Improve Patient Outcomes: Lessons Learned After a Decade of Clinical Transformation

Alternative payment models (APM) incentivize providers to keep patients healthy and proactively improve outcomes through preventive care and care coordination. As the cliche goes, they encourage quality over quantity and value over volume. Accountable care organizations (ACOs) are the leading APM in Medicare and focus on more than just creating savings by removing inefficiencies from the health system. In this virtual panel, some of the most successful ACOs will share how they’ve transformed clinical practices to improve patient care. Examples include the use of care coordinators, smarter use of home health and post-acute care, data analytics, an emphasis on transitions of care, and engaging specialists in care models.
