The deadline for clinician practices to apply to participate in the Primary Care First (PCF) Model is Wednesday, January 22. Practices can create an account and submit an application through the Primary Care First application portal.
All payers seeking to align with the model should complete an online proposal by March 13, 2020. Payers interested in partnership in multiple regions are asked to submit separate proposals for each region if their proposed approach varies significantly between regions.
The PCF Model is a set of voluntary payment options providing payment flexibility and enabling practitioners to provide advanced primary care services. It is designed as a multi-payer model and CMS is seeking payer partners that can align with the Medicare PCF payment methodology, quality measurement strategy and data sharing approach.
