New Case Study Published for the Accountable Health Communities Model

The Accountable Health Communities (AHC) Model tests whether the health-related social needs of Medicare and Medicaid beneficiaries will impact health care costs and reduce health care utilization.

The Centers for Medicare and Medicaid Services just published a case study which describes a successful navigation approach from one bridge organization that could inform practice at another AHC Model site.
