CMS Solicits Applications from Practices and Payers for Round 2 of CPC+

The Centers for Medicare and Medicaid Services (CMS) is soliciting applications from practices and payers for Round 2 of the Comprehensive Primary Care Plus (CPC+) model. Four regions were selected for Round 2:

  • Louisiana: Statewide
  • Nebraska: Statewide
  • North Dakota: Statewide
  • New York: Greater Buffalo Region (Erie and Niagara Counties)


Applications are being accepted until July 13, 2017.


CPC+ is a national advanced primary care medical home model that aims to strengthen primary care through regionally-based multi-payer payment reform and care delivery transformation. CPC+ includes two primary care practice tracks with incrementally advanced care delivery requirements and payment options. In 2017, CPC+ Round 1 began in 14 regions, with 53 payers and 2,891 practices.
