CMS Shares Early Insights from the Accountable Health Communities Model

The Centers for Medicare & Medicaid Services (CMS) released a summary document of key takeaways from a meeting on the Accountable Health Communities (AHC) model. The AHC model is designed to assess whether bridging the gap between clinical care and social services can reduce healthcare utilization and costs for high-risk Medicare and Medicaid beneficiaries. In November 2019, CMS convened 29 participating bridge organizations and key partners for the second annual AHC meeting to discuss strategies to address beneficiaries’ health-related social needs.
